After More Than 30 Companies, Derik Fay Is Betting That Infrastructure Outlasts Hype

Fast-growing markets create a peculiar kind of pressure.

Everything tells the entrepreneur to move faster.

Customers are arriving.

Competitors are appearing.

Capital is looking for somewhere to go.

Headlines are getting bigger.

The obvious instinct is to chase the thing everyone wants today.

Derik Fay is trying to do something different.

After building, scaling, investing in or exiting more than 30 businesses across several industries, Fay has returned to day-to-day operations through RX Management Systems. His timing places him directly inside the GLP-1 and peptide boom, but his strategy is built around a more cautious assumption.

The medication generating today’s excitement will not necessarily be the medication generating tomorrow’s excitement.

What survives is the infrastructure around it.

That idea reflects the unusual path Fay took before returning to the CEO seat.

Through 3F Management, founded in 2004, he spent years operating businesses across fitness, financial services, payment processing, marketing, technology and other sectors.

Around the Clock Fitness became one of his most recognizable companies. Fay founded it in 2006, expanded the business and ultimately saw it acquired by a Nasdaq-listed company in 2019.

After that exit, he did not immediately start over.

He moved closer to the institutional side of business.

For approximately five years, Fay advised Fortune 500 leadership teams on mergers and acquisitions, restructuring, governance and strategy while 3F Management increasingly focused on investments, equity partnerships and joint ventures.

The experience reinforced a lesson that founders sometimes learn too late.

The fastest-growing company is not automatically the strongest company.

Durability comes from what sits underneath growth.

That is the lens Fay is applying to healthcare.

RX Management Systems has been designed around more than pharmaceutical demand. The organization combines management services, pharmacy relationships, fulfillment, acquisitions, compliance and strategic growth.

Believe RX provides the pharmacy component. Specialty Care handles fulfillment and operational logistics. RX Management Systems serves as the broader management platform.

That structure reflects Fay’s belief that the highest-value opportunity may eventually sit around the medication rather than inside the medication itself.

There is precedent for that idea throughout business.

Products change faster than infrastructure.

Individual technologies come and go while the networks supporting them remain.

Payment companies survive changes in consumer products because transactions still need to move.

Logistics companies survive changes in retail because products still need to be delivered.

Cloud infrastructure remains valuable even when the applications running on top of it change.

Fay sees healthcare through a similar lens.

GLP-1 created the immediate opening.

But the platform is supposed to remain relevant as therapies, regulations and consumer preferences evolve.

That requires discipline because infrastructure is rarely the most glamorous part of a boom.

Compliance does not generate the same excitement as explosive consumer demand.

Fulfillment is less interesting than a breakthrough medication.

Management systems do not dominate headlines.

Yet those are often the areas that determine whether rapid growth becomes sustainable growth.

Fay has made regulatory infrastructure particularly important.

His approach assumes that oversight surrounding fast-growing healthcare categories will increase as the market matures.

Rather than treating regulation as something to respond to later, the company has focused on multi-state operating considerations, centralized fulfillment and compliant prescription routing as part of its broader architecture.

The philosophy resembles how Fay approaches acquisitions.

Think about tomorrow’s problem while it is still inexpensive enough to solve today.

That mentality may be the clearest through-line connecting the different phases of his career.

Early in his career, Fay learned how to build.

Exits taught him how companies are valued.

Payment processing exposed him to infrastructure.

Advising larger corporations showed him how sophisticated organizations evaluate risk.

Investing taught him what happens when the founder is no longer the only person making decisions.

RX Management Systems brings those experiences together.

There is no guarantee that GLP-1 will remain the defining healthcare story indefinitely.

Fay’s strategy does not require it to.

His larger wager is that whatever comes after GLP-1 will still need pharmacies, fulfillment, technology, compliance, payments, management and capital.

Trends create attention.

Infrastructure creates something companies can potentially build on long after the attention moves somewhere else.
